Pursuant to Article 6(1)(b) of Regulation (EU) 2016/679 (hereinafter: the "Regulation"), I consent to the processing of my personal data in the process of recruitment for the position for which I am applying or a position requiring similar qualifications. My consent covers the entire stage of the recruitment process announced and conducted by Dr. Dominik Matczak, doing business under the name SILVERHAND Dominik Matczak (ul. Garbary 35/9, 61-868 Poznań, employment agency registered in the KRAZ register under No. 7822), who is also the Administrator of the personal data (hereinafter: "Silverhand" or "Administrator"). I am aware of the fact that the recruitment process in which I am participating is conducted on behalf of a potential employer located in Poland or in the territory of the EU/EEA, who has contracted Silverhand to perform the service. I also take this opportunity to give my consent for the purposes of future recruitment processes conducted over a period of 7 years from the date of submission of my application documents, with the exception of a situation where the recruitment contract will be further executed or the Administrator will be obliged to process (including storing) the data on the basis of generally applicable laws. I agree to the transfer of personal data specified in Article 22 (1) § 1 of the Labor Code (name, surname, date of birth, contact data indicated by me, including telephone number, e-mail address, education, information on professional qualifications and course of previous employment). Voluntarily and on my own initiative, I also agree to the processing of personal data referred to in Article 22 (1) §3 of the Labor Code, as well as the following information belonging to a special category of personal data within the meaning of Art. 9 of the Ordinance: address of residence or registered address, PESEL number, series and number of identity card, all information contained in the document of identity card, driver's license, or other documents confirming my other skills, marital status, number and data of children, number of bank account to which the future employer will transfer remuneration for work, photo showing my image, and information on my health condition. However, I would like to emphasize that I am aware that at the stage of recruitment, neither Silverhand, nor the future or potential employer can require me to give such consent (a special category of data), or make the outcome of the recruitment contingent on giving it. I understand and acknowledge that the lack of consent to the processing of personal data or its withdrawal cannot be the basis for unfavorable treatment of an applicant for employment, and cannot cause any negative consequences to him, in particular, cannot be a reason justifying the refusal of employment, termination of the employment contract or its termination without notice by the employer. I also undertake not to provide Silverhand with my personal data regarding convictions and violations of the law within the meaning of Article 10 of the Ordinance, regardless of whether or not I have a prior criminal record. I acknowledge and agree that Dr. Dominik Matczak authorizes all persons employed by him on the basis of an employment contract, contract of mandate, contract for specific work or managerial contract to process my personal data (the list of these persons can be found on the website www.silverhand.eu - in the tab "About us" and is limited to branches only: Poznań, Ostrów Wielkopolski). At the same time, I know that I have the right to access the content of my personal data and to correct them, to withdraw my consent to their processing at any time, which I can exercise by sending the appropriate request to
